Do not use our appointment booking form to change or correct an existing birth, marriage or death registration. There is a separate process for this.
You must first complete the relevant form below and email it to register.office@calderdale.gov.uk. Forms must be printed and signed in ink. If you are unable to print a form, please let us know and we will post a copy to your home address.
