• create a request for a service;
• and for us to collect statistical information for reports.

If you complete a form and share your information with us

This constitutes consent from you for us to process your data for the above purposes.

You have the right to

• See what information is held about you.
• Have inaccurate information corrected.
• Have information removed from our system.

You must make the above request in writing and email to: Customer.First@Calderdale.gov.uk.

If you feel your data has not been handled lawfully

You have the right to complain to our Data Protection Officer. You can contact the Council’s Data Protection Officer at information.management@calderdale.gov.uk.

How we handle you details

• Your name, address and contact details are stored on our system to keep up-to-date records.
• They will be kept for a maximum of 12 months from the date of your service request.
