Apply for a Blue Badge
Use this form to apply for a disabled parking badge so you can park nearer to some shops and facilities.
Before you begin, it would help to have these to hand:
- A recent digital photo that shows your head and shoulders.
- Photo or scan of your proof of:
- Identity, such as a birth certificate, passport or driving licence.
- Address, such as a Council Tax bill or government letter.
- Benefits (if you get any).
- National Insurance number (if you have one).
- Details of your current Blue Badge. (Only for when you reapply).
