Apply for home to school transport for your child

Before you begin, you will need these details to hand:

• Previous address and date of move (if you moved house in the last 12 months).
• Name of the school you want the pass for.
• Date you want the pass to start

This form can be used to apply for:

• A zero fare bus pass via our Citizen Portal.
