This form can be used to:
- Book an appointment to register the birth of a child, or children in the case of twins or triplets.
This form cannot be used to:
Before you begin, it would be helpful to have:
- Child's date and place of birth.
- The forename(s) and surname in which it is intended that the child will be brought up.
- Mother's name, address, date and place of birth, and occupation.
- Father or second parent's name, address, date and place of birth, and occupation (optional).
