This form can only be used to apply for a clinical waste collection for:

• Home dialysis waste.
• Used bloodlines.
• Dressing waste.
• Feed tube.
• Stoma bags.
• Colostomy bags.
• Incontinence waste.
• Needles.
• Syringes.

Note: Please make sure that all your waste is:

• tied up securely;
• and is in a place that can be easily accessed, ready for collection.
