It identifies the priorities for action to improve health and wellbeing and reduce health inequalities.
It is used to:
- Develop the Calderdale Health and Wellbeing Strategy.
- Inform the commissioning of health and care services.
- Inform the development of community assets and building blocks of good health.
It links closely with Vision 2034 and will also be used to underpin the development of Calderdale Cares.
