How we use your information

The type of information we will collect and process

The following information will be collected directly from you, your family member, or your carer:

  • Name, address, telephone number(s)
  • Date of birth
  • GP Surgery
  • Health details
  • Details of the support being given/received
  • Carers personal circumstances such as any children living at home and any help they give with care, work/family commitments, relationship to the person being cared for, details of anyone else you provide care for

The purpose of processing your information

Your personal information is collected for the following purposes:

  • To comply with our duty to undertake a Carers Assessment under the Care Act (2014)
  • To assess whether the carer has needs and what those needs may be, to consider the impact of the caring on the carer, and whether the carer’s needs meet the eligibility criteria for support from the Council
  • To agree a support plan, which sets out how the carer’s needs will be met
  • To communicate with you to provide appropriate information and updates
  • To administer the making of any payments via the Council’s Prepaid Card system.

The lawful basis for the processing of your information

Personal information is processed under the following legal basis:

  • Article 6(1)(c) – for compliance with a legal obligation to which the controller is subject, to keep financial records regarding any payments made which the controller is subject under legislation including the Companies Act (2006)
  • Article 6(1)(e) - for the performance of a task carried out in the public interest or in the exercise of official authority vested in the controller under legislation including the Care Act (2014).

Special Category Information is processed under the following conditions:

  • Article 9(2)(a) – Explicit consent
  • Article 9(2)(h) – The provisions of health or social care or treatment and the management of health and social care systems and services.
  • Schedule 1, part 1, DPA 2018 (2) – Health or social care purposes

Sharing your personal data

Your personal information will be shared with the following organisations or services commissioned by Calderdale Council, where appropriate and consent has been given:
