Calderdale Council is registered with the Information Commissioner’s Office (ICO) under the provisions of the Data Protection Act 2018. The Council takes its responsibilities under the Act very seriously.
The information provided by you is collected to enable us to acknowledge your donation, if acknowledgment is requested, to confirm your agreement to our acceptance criteria, to enable us to maintain accurate provenance of the item(s) donated and their legal ownership by Calderdale Council and, if applicable, maintain accurate copyright information.
Completion of this form/sharing your information with us constitutes explicit consent from you for us to process your data for this purpose. You have the right to see what information is held about you and to have inaccurate information corrected, by contacting: libraries@calderdale.gov.uk.
You have the right to complain to the Data Protection Officer if you feel that your data has not been handled in accordance with the law. You can contact the Council’s Data Protection Officer at information.management@calderdale.gov.uk.
The form, as completed by you, will be filed in hard copy in an area of the library accessible only to staff and an electronic copy will be held securely within the Council’s network. This information will be kept indefinitely or until such time as the data is reviewed by us.
