We need to collect this information in order to act responsibly in safeguarding valuable Local Studies materials.
Sharing your information with us constitutes explicit consent for us to keep it for this purpose.
This form, on which you have entered your name and address, will be destroyed upon your return of the items listed to a member of Local Studies staff.
