This privacy notice explains how your personal information will be processed if a referral is made to Calderdale Council’s Shared Lives service and in relation to Adult Social Care services. The Shared Lives service offers short breaks, long-term care and daytime support for people who need extra help in their lives and offers support to Shared Lives Carers.
We collect the following types of information from you directly or from third parties such as referrers, service providers, and the NHS. Information may be collected in many ways including on paper, by telephone, electronically or by a member of staff in person:

Local health and social care services are working together to improve how your information is shared to support the decisions they make about your care. We ask for your NHS Number as it helps to uniquely identify you when sharing information between health and social care. Using the same number improves our ability to work together to improve your care and support.
The Yorkshire & Humber Care Record is a shared system that allows health and care staff within the Yorkshire & Humber Health and Social Care community to appropriately access the most up-to-date and correct information about patients, to deliver the best possible care.
The National Data Guardian opt-out programme is a service that allows people to opt out of their confidential patient information being used for research and planning.
