• Child's address details and how long they have lived at their current address.
• Name of the school you were offered (if applicable).
• Names of your preferred schools (up to 5 schools).
• The grounds of your appeal.
• Any additional documents you want to upload to support your appeal.

This form can be used to:

• Appeal a school place if you have received formal notification that your request for a place at your preferred school has been declined.
