- Child's address details and how long they have lived at their current address.
- Name of the school you were offered (if applicable).
- Names of your preferred schools (up to 5 schools).
- The grounds of your appeal.
- Any additional documents you want to upload to support your appeal.
This form can be used to:
- Appeal a school place if you have received formal notification that your request for a place at your preferred school has been declined.
