- National insurance number for yourself and your partner.
- Details of any benefits you and/or your partner are receiving.
- Name, address and start dates of any employment for yourself and/or your partner.
- Details of any accounts, savings or investments that you and/or your partner hold.
- Details of the loan amount and repayments you and/or your partner are receiving.
- Details of any other person(s) living in your home.
This form can be used to:
- Review the amount of housing benefit being received.
- Review the amount of council tax benefit being received.
