SEND early identification and notification form

This form is for health practitioners to notify Calderdale Council if a child has, or is likely to have Specialist Educational Needs (Section 23).

This form is only to be used by, or on behalf of, qualified health professionals.

Integrated Care Boards (ICB), NHS Trusts and NHS Foundation Trusts MUST inform the appropriate Local Authority (LA) if:

  • They identify a child under compulsory school age as having or probably having a disability or special educational need. (Section 23 of the Children and Families Act 2014).

Note: A child reaches compulsory school age the term after their fifth birthday.

Before you begin, it would help to have these details to hand:

  • Details of the child, including NHS number and medical condition/diagnosis.
  • Relevant reports and details of the reason for notification to upload.

The purpose of this form: 

  • Advise the local authority of a preschool child who is likely to have a specialist educational need and/or disability.
  • The information will support the planning of services and future commissioning countywide.

Please note: A section 23 notification is not a referral for further services.
