You can use our Wellbeing Contact Form to connect with us if you or someone you know requires advice, information or support around a social care matter. Upon receipt, we will make contact with you within 7 working days.

Before you begin, it would be helpful to have these details to hand:

• If this request is on behalf of another person please include his/her name, address and age, relationship to you.
• GP name (if known).

This form can be used to apply for:

• Requesting information about adult care services
• Asking for an assessment of need.
